Application Icon

Just a minor problem, the icon displayed for the main application within a install package is incorrect. It seems to be random, I have 7 install versions of the same application and each icon for the core app is different in each list. Any one else have this problem?

Where is this icon being shown? Shortcut? Add/Remove Programs panel? Can you please provide some screen shots and eventually a sample AIP/MSI to reproduce the problem?
