Is there a possibility to add custom form where user will decide if he wants to perform some action during install (custom action) ?
e.g. During instalation I want to launch external .exe base on user preference
tx a lot in advance

I'm afraid you cannot add such a dialog directly in AI. However here are two alternatives:
1. If you have access to the source code of the executable then the program itself should ask the user if he/she want to execute it.
2. Achieve this with two custom actions. The first displays a dialog asking the user and based on it's response sets a property. This property can then by used as a condition to launch the second CA that is the EXE itself.
