Desktop shortcut, remove triggers?

I'd search for this if I could, I know it's been asked before.

First question - how do I drop a shortcut onto the desktop?

Second, I'm still learning how to ride Advanced Installer. I'm falling down a bit. A customer in the field with a test installer was prompted to remove the existing installation (a bad thing). What exactly determines when the installation process will request the removal of a previous installation?
