it was the corporate antivirus that was deleting the file when it was created

I am glad to hear you managed to find the culprit.
Most likely, we are talking about a false-positive detection here
What I would suggest is submitting the setup for whitelisting to the AntiVirus vendor you are using and that should get rid of the issues.
